This newspaper article explains the chain of events
In fact the EA Board Paper - November 2003 was amended prior to circulation. The final document (including embedded amendments) was then published on the internet. Enabling amendment tracking thus allowed the changes to be identified.
For example, Barbara Young inserted the following amendments in red at 6.24pm on 13 November 2003
- Concern
that the channel capacity does not meet the design criteria.
Resulting potential liability issues
